Issues with on demand content - Time Warner TV

This question is about "Issues with on demand content", with Time Warner Cable TV and apps. I'm having issues with on demand content. Where to begin......

-- occasionally no sound when staring a program. Have to pause it and then press play to get sound back

-- occasional pixelation

-- occasionally no video. Have to hit pause and then play

-- occasionally content gets "stuck" on an ad. Again, I have to hit pause and then play
