OTA Broadcast Antenna Search Tool on Website

I would like to have a customized OTA Broadcast Antenna Search/Locator Tool on my website. You see them on all the antenna sales and streaming box websites. I've conducted several web searches with no success. Any ideas on how this is done? Thanks!
